Frank and Ann have been happily married for 65 years. They have done everything together since the day they met while swimming at the Irving pool in Allentown, Pennsylvania.

Throughout the years, they have both had some health complications. They have cared for each other with help from their daughter LeeAnn, who resides in Delaware. During a routine phone call, LeeAnn noticed her dad wasn’t speaking clearly, so she called 911. Frank was admitted to Lehigh Valley Hospital - Cedar Crest. Only days after Frank was hospitalized, Ann fell at home and was admitted to St. Luke's Hospital - Allentown Campus. Since Ann and Frank were in different hospitals receiving treatment, Ann’s doctors were exploring options for inpatient therapy. Ann spoke up and said she had received “wonderful care and attention” back in 2021 while at ProMedica Skilled Nursing and Rehabilitation located in Allentown. Since LeeAnn was supporting both her mom and dad in different hospitals, she was anxious to have them together. ProMedica was their place of choice to recover together.

Frank and Ann were able to room and rehab together, just like they wanted. They received physical, occupational and speech therapy. ProMedica allowed them to support and care for each other, while receiving the care they each needed. They were able to successfully reach their goals and were discharged home with assistance from LeeAnn and home health care. Ann and Frank were anxious to get home to their beloved dog Maggie and home cooked meals.

Patient Profile: 

After a serious accident, Kathleen was admitted to the hospital. She was was severely injured with several broken bones that required surgery. Post-surgery, Kathleen was unable to walk or transfer and was in need of rehabilitation and nursing care before returning home.

Post-acute Need:

Kathleen, 74, lives on her own and gardens extensively. She required physical and occupational therapy to help get her strong enough to transfer and walk on her own. She was in severe pain and had surgical incisions that required care.

Post-acute Stay:

At the center, the interdisciplinary team created a care plan to help Kathleen meet her goals. The nursing team helped Kathleen manager her pain and taught her about caring for her incisions. The therapy team helped Kathleen work on strengthening, balance and range of motion so she would feel confident returning home. The team did a home assessment to ensure Kathleen was confident in her ability to get around on her own.
