What To Expect During Your Stay
Post-Hospital Care is designed to help you return home or to a lower level of care after a hospital stay. It begins with an interdisciplinary staff including nurses, therapists, therapy assistants, dietitians, social workers and recreation staff—all charged with helping guide your progress, manage your discharge planning and reduce the probability of return to the hospital.

Arrival
After getting settled in your room, you will be evaluated by an interdisciplinary team. We will work with you to define your individual goals and outline your care plan. Your plan will include your treatment regimen, medication, nutrition, activities of daily living, priorities, scheduling preferences and circumstances.
While You Are There
Throughout your stay, our team will monitor your care and report to your physician and managed health care plan. You can expect to receive therapy in our hospital-level therapy gyms. We also offer a proactive approach in mental wellness, pain management intervention and wound management.
Discharge
You will be assigned a discharge planner who will work with your therapy team to ensure you are able to perform daily tasks once you return home. We will coordinate all follow up appointments for you before you leave to go home.
Follow Through
After you return home, we will call you to see how things are going. We will answer any questions you may have after transitioning home and make sure everything is on track for your follow up appointments.
Customized Care
The expertise of the clinical and rehabilitation team can significantly impact your recovery success. From the moment of your admission at MedBridge, you’ll be evaluated by an interdisciplinary team, including a certified rehabilitation and nursing staff trained to meet your special needs.
Your team may include:

- Case manager
- Consulting physicians and physiatrist
- Dietitian
- Hospital/Nurse liaison
- Nurse practitioner
- Physical and occupational therapists
- Social worker and discharge planner
- Speech and language pathologists
Together, your team will create a regimen tailored just for you. You’ll use leading-edge equipment to aid in your rehabilitation.
Throughout your stay, our team will monitor your care and report to your physician and managed health care plan. For return home, we may perform a home evaluation if needed and arrange for equipment and home care needs.

- Rooms are all private or semi-private
- Short-stay population is 90 percent or better
- Small and personal, ranging between 20 and 60 beds
- Staffed by registered nurses
Payment Options
MedBridge units are Medicare-certified. In addition, we accept most managed care and commercial insurance, as well as private payment. Many of our centers also accept Medicaid.
